Can we run a gift with purchase where the free item must match the size the customer already has in the cart?

Not as a single automatic rule. Matching the variant of the gift to the variant already in the cart is not supported end to end. The recommended workaround is a "Item discount with a specified quantity" rule combined with frontend logic that lets the customer choose and add the correct gift variant themselves.
