Can a refund be reverted if it was made on the wrong item?

No. A refund cannot be recalled once it has been made. If the wrong amount reached the customer, contact your payment provider to see what can be corrected on their side. Leave a comment on the order in the Merchant Portal describing what happened, so the next person to open the order understands the history.
